Items are grouped in two subdimensions, each composed by five items: PhH and PsH. Questions refer the respondent to the previous month. The responses on a four-point Likert response format, are ‘never’ (score = 0), ‘sometimes’ (score = 1), ‘most of the time’ (score = 2) and ‘all the time’ (score = 3). A ‘not assessable’ column was included in the parent version of the questionnaire to designate questions that cannot be answered because of developmental immaturity. |
