52 items organized in 3 sections and 12 subscales.   The four activity subscales have been aggregated into a total General Activity scale.  The response options and 3 overall scales. Each item is rated on a 0-6 scale, and the scores for each subsale are calculating by adding scores for each item in that subscale (divided by the number of items)
